I am interested in buying a boat I recently saw advertised.
The  boat has been sitting for three years. The owner has had medical problems. It is a 1989 tracker TF with a V150 XR-4 Mercury. He said last time he had it out everything worked perfect.  It currently has 10 gallons of old gas that needs to be drained.
How much would it cost to get the engine tuned up and the gas drained?

Is this boat at risk since it has been sitting for three years?

First of all the gas has to come out.  If you have the money and the owner will allow the boat to be checked out, take it to a reputable dealer of YOUR choice and pay to have it drained and the motor checked out completely.  That means, compression, etc.  Have them double check all of the wiring and if it has a live well, check it out also for the fittings, etc.  Electronics check along with the motor, is very important.  Also, check out the hubs on the trailor as well.  Some of it you may be able to check yourself without the dealer, just make sure your satisfied before the purchase, afterall, it's your money.

Chuck is right on....wiring will corrode...fuel lines will deteriorate and any on board water will rot....if there is any wood at all I would bag it.

He is asking 300 above blue book for it or best offer. It needs 2  deep cycle batteries, 2 new trailer tires, tune up, and gas drained.

If I get it checked out and decide on it how much below blue book should I offer since it has these issues i have to fix? Do you think it is worth it?

I found the same "good deal" two years ago a 90' nitro for $3500 that sat for three years. I soon found out that a boat that has sat un-used is bad news, add about $2000 to the cost. Then plan on selling it in a year so you can buy a boat that you can fish out of not fix.

bought my boat the same way about 5yrs ago , had a few problems but not big ones.I have the 185TF 90 and its great. not brand new but does 65 and 66 on good day. thats with a 4 blade, be careful of corse but dont rule it out. is that frost's boat?

Not Frosts boat. It currently belongs to a Boeing retiree who had serious medical problems. I guess he just used it for panfish and pulling his grand kids.

Honestly, I know squat about nitro boats, only that we have had club members go through them in the past.  What I do like is the Merc.  Likely a black max.  Tough runner and pretty darn reliable.  A new fuel pump and a good carb cleaning along with a lower unit service, including water pump, BEFORE you even start it, and you should be good too go.  Good luck.
